This Quotation Estimate is issued by the Kyoto Mind & Wellbeing Centre to formally outline the scope, schedule, and associated costs of professional Psychologist services to be delivered within the Japan Kyoto metropolitan area. This document serves as a binding financial proposal and does not constitute a final contract until countersigned by both parties. All rates quoted herein are exclusive of the applicable 10% Japanese Consumption Tax (JCT), which will be added at the point of invoicing. The Psychologist named in this Quotation Estimate holds full licensure under Japanese national regulations and is registered with the Japan Psychological Association, ensuring compliance with all ethical and professional standards governing psychological practice in Japan Kyoto and the broader Kansai region.
The following services are encompassed under this Quotation Estimate. Each service is to be performed by the designated Psychologist or, where noted, by a supervised associate psychologist under the direct oversight of Dr. Tanabe. All sessions will take place at the Kyoto Mind & Wellbeing Centre premises in Higashiyama, Japan Kyoto, unless a remote telehealth arrangement is mutually agreed upon in writing.

* All amounts are denominated in Japanese Yen (JPY). For international clients, equivalent amounts in USD or EUR may be provided upon request at the Bank of Japan reference rate on the date of invoice issuance.
The following terms govern the acceptance and execution of this Quotation Estimate for Psychologist services in Japan Kyoto:
- Validity: This Quotation Estimate remains valid for a period of ninety (90) calendar days from the date of issue. Beyond this period, all rates are subject to revision based on prevailing market conditions and the Psychologist's updated fee schedule.
- Payment Terms: A 30% deposit of the total Quotation Estimate value is due upon acceptance. The remaining balance shall be invoiced monthly in arrears, with payment due within fourteen (14) days of invoice receipt. Late payments will incur a 1.5% monthly surcharge.
- Cancellation Policy: Sessions may be rescheduled or cancelled with a minimum of 48 hours' written notice. Cancellations made within 48 hours will be charged at 50% of the session fee. No-shows will be charged in full.
- Confidentiality: The Psychologist is bound by the Japanese Act on the Protection of Personal Information (APPI) and the ethical code of the Japan Psychological Association. All client records, session notes, and reports generated under this Quotation Estimate will be stored securely and will not be disclosed to third parties without written client consent, except where mandated by Japanese law.
- Scope Limitations: This Quotation Estimate covers psychological assessment, therapy, and consultation services only. It does not include psychiatric medication management, which would require a separate referral to a licensed psychiatrist in Japan Kyoto. The Psychologist will coordinate such referrals at no additional administrative cost.
- Force Majeure: Neither party shall be liable for failure to perform obligations due to natural disasters (including earthquakes, typhoons, or volcanic activity common to the Japan Kyoto region), government-mandated closures, or other events beyond reasonable control.
- Governing Law: This Quotation Estimate and any resulting agreement shall be governed by and construed in accordance with the laws of Japan. Any disputes shall be resolved through mediation in Kyoto-shi, Japan, before submission to the Kyoto District Court.
By signing below, the client acknowledges receipt of this Quotation Estimate, confirms understanding of the scope of Psychologist services to be provided in Japan Kyoto, and agrees to the terms and conditions set forth herein. The Quotation Estimate shall become a binding service agreement upon countersignature by both parties and receipt of the initial deposit.
