Terms and Conditions of This Quotation Estimate
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. After the expiration date, all rates listed herein are subject to revision based on current market conditions in the Japan Tokyo professional services sector.
- All services described in this document will be performed by a duly licensed and registered Psychologist in accordance with the Japan Registered Psychologists Act (Act No. 117 of 2003) and the ethical guidelines of the Japanese Association of Applied Psychology.
- Confidentiality: All psychological data, session notes, and assessment results collected during the engagement in Japan Tokyo will be handled in strict compliance with Japan's Act on the Protection of Personal Information (APPI, 2020 revision). No individual employee data will be disclosed to the client without written consent.
- Payment Terms: A 40% advance payment is due upon acceptance of this Quotation Estimate. The remaining 60% shall be invoiced in two equal monthly installments. All payments are to be made via bank transfer to the account designated by Shinjuku Mind & Wellness Center, Japan Tokyo.
- Cancellation Policy: Should the client wish to cancel or modify any scheduled Psychologist session, a minimum of 48 hours' written notice is required. Cancellations within 48 hours will be subject to a 50% fee of the scheduled session rate.
- Scope Limitation: This Quotation Estimate covers psychological counseling, assessment, and organizational consulting only. It does not include psychiatric medication management, emergency hospitalization, or legal representation. Should a Psychologist determine that a client requires referral to a psychiatrist or other specialist in Japan Tokyo, such referral will be provided at no additional cost, but subsequent specialist fees are not included in this estimate.
- Force Majeure: Neither party shall be liable for failure to perform obligations due to natural disasters, government mandates, or other events beyond reasonable control, which are particularly relevant given the seismic and typhoon activity in the Japan Tokyo region.
- Governing Law: This Quotation Estimate and any resulting contractual relationship shall be governed by and interpreted in accordance with the laws of Japan. Any disputes shall be resolved through mediation in Tokyo, Japan, before resorting to litigation in the Tokyo District Court.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and fee structure outlined above for the engagement of a licensed Psychologist to deliver the specified mental health services within Japan Tokyo.
