3. Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for thirty (30) days from the date of issuance. After the expiration date, all rates, availability, and terms herein shall be subject to re-evaluation by the Psychologist and the center's administration.
Payment terms: Fifty percent (50%) of the total amount shall be due upon acceptance of this Quotation Estimate as a deposit. The remaining fifty percent (50%) shall be settled within fifteen (15) business days from the completion of all services.
All sessions and consultations shall be conducted at our primary office located in Philippines Manila (Makati City) unless otherwise agreed upon in writing. Travel to other locations within Metro Manila may incur an additional transportation surcharge of ₱500.00 per trip.
The Psychologist reserves the right to reschedule appointments with a minimum of twenty-four (24) hours' prior notice. Cancellations by the client within twelve (12) hours of a scheduled session shall be subject to a 50% session fee.
All psychological records, test results, and reports generated during the engagement are strictly confidential in accordance with the Philippine Data Privacy Act of 2012 (RA 10173) and the Code of Ethics of the Psychological Association of the Philippines.
This Quotation Estimate does not constitute a guarantee of specific therapeutic outcomes. The Psychologist will employ evidence-based interventions appropriate to the client's diagnosed or presenting condition.
Any additional services not explicitly listed in this document shall be subject to a separate Quotation Estimate and written approval prior to commencement.
Disputes arising from this engagement shall be resolved through amicable negotiation. If unresolved, matters shall be referred to the appropriate courts of Philippines Manila or through the Philippine Arbitration Law (RA 8762).

Important Note: This Quotation Estimate is prepared exclusively for the client named above and may not be transferred, copied, or used for any other purpose without prior written consent from Manila Psychological & Wellness Center, Inc. The Psychologist named in this document is the primary provider; in the event of unavailability, a qualified substitute Psychologist of equivalent credentials based in Philippines Manila may be assigned with the client's prior approval. 4.
