1. Professional Standards: The Psychologist engaged under this Quotation Estimate shall adhere to the Swiss Federal Act on the Free Movement of Health Professionals (FMH) and the professional code of ethics established by the Swiss Society for Psychology. All sessions are conducted in strict confidentiality in compliance with the Swiss Federal Act on Data Protection (FADP / nLPD).
2. Payment Terms: Payment of the total amount specified in this Quotation Estimate is due within fourteen (14) days of the completion of the final service session. Invoices will be issued in CHF and may be settled via bank transfer to our account at UBS AG, Zurich (IBAN: CH93 0023 0456 7890 1234 5). A late-payment interest of 5% per annum shall apply to overdue balances.
3. Insurance and Reimbursement: Clients holding Swiss basic health insurance (KVG/LAMal) may be eligible for partial or full reimbursement of psychological services. Our Psychologist will provide all necessary documentation, including the official treatment plan and itemized invoices, to facilitate claims with the client's insurer. The client is responsible for any co-payment (franchise) or non-covered portions.
4. Cancellation Policy: Sessions must be cancelled or rescheduled at least 48 hours in advance. Cancellations made within 48 hours or no-shows will be billed at 50% of the session fee as stated in this Quotation Estimate.
5. Scope Limitations: This Quotation Estimate covers the services explicitly listed above. Any additional sessions, specialist referrals, or ancillary services not included herein will be subject to a separate written agreement and supplementary quotation.
6. Jurisdiction: This agreement is governed by the laws of the Canton of Zurich, Switzerland. Any disputes arising from this Quotation Estimate shall be resolved by the competent civil courts of Switzerland Zurich.
