- This Quotation Estimate is valid for a period of ninety (90) calendar days from the date of issue. Should the client wish to proceed after the expiry date, a revised Quotation Estimate reflecting any updated fee structures will be issued by the Psychologist or their designated administrative representative.
- All services will be conducted at the Birmingham Psychological Services Clinic located in the heart of United Kingdom Birmingham, or via secure video link for remote sessions. The Psychologist reserves the right to relocate sessions to an alternative accessible venue within Birmingham upon 48 hours' written notice.
- Payment is due within fourteen (14) calendar days of the date of this Quotation Estimate or the date of the final invoice, whichever is later. Payment may be made by bank transfer, debit/credit card, or direct debit. Late payments will incur interest at a rate of 4% above the Bank of England base rate, in accordance with the Late Payment of Commercial Debts (Interest) Act 1998.
- The Psychologist is bound by the HCPC Code of Conduct and Ethics, the BPS Code of Ethics and Conduct, and the UK General Data Protection Regulation (UK GDPR) and Data Protection Act 2018. All client records will be stored securely in compliance with these regulations.
- Cancellations must be provided with a minimum of forty-eight (48) hours' notice. Sessions cancelled with less than 48 hours' notice will be charged at 50% of the applicable session fee. No-shows will be charged in full.
- This Quotation Estimate does not constitute a contract until formally accepted in writing by both parties. Acceptance may be communicated by signing the acknowledgment section below or by written email confirmation referencing the Quotation Estimate number QTE-2025-BHM-00472.
- The Psychologist may, at their clinical discretion, recommend additional sessions or services beyond those itemised in this Quotation Estimate. Any such additional services will be subject to a supplementary Quotation Estimate and the client's prior written consent.
- All disputes arising from this Quotation Estimate or the services described herein shall be governed by the laws of England and Wales and shall be subject to the exclusive jurisdiction of the courts of United Kingdom Birmingham.
- The Psychologist carries professional indemnity insurance with a minimum cover of £5,000,000 per claim, maintained with a recognised UK insurer. A certificate of insurance may be provided upon request.
