1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing for the Psychologist services listed herein may be subject to revision based on market conditions in Uzbekistan Tashkent.
2. All Psychologist services are to be delivered at the Psyche Counseling Center premises located at 42 Amir Temur Avenue, Tashkent, Uzbekistan, unless otherwise agreed in writing for on-site or telehealth delivery.
3. Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% due upon completion of the final session. Payment may be made via bank transfer to the account of Psyche Counseling Center, JSC, at the National Bank of Uzbekistan, or by cash at the office in Tashkent.
4. Cancellation policy: Sessions may be rescheduled with a minimum of 24 hours' notice. Cancellations made less than 24 hours before a scheduled Psychologist appointment will incur a 50% fee of the session cost.
5. Confidentiality: All client information, session notes, and psychological reports are protected under the Law of the Republic of Uzbekistan "On Personal Data Protection" (2019) and the professional ethical code of the Uzbekistan Psychological Association. No information will be disclosed without written consent.
6. The Psychologist assigned to this engagement holds a Master's degree in Clinical Psychology from Tashkent State University of Medicine, is registered with the Ministry of Health of Uzbekistan, and maintains a minimum of 8 years of professional practice in Tashkent.
7. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties. Acceptance may be indicated by signing the acknowledgment section below or by issuing a purchase order referencing Quotation No. QE-2025-0347.
8. All services are subject to the availability of the assigned Psychologist. In the event of unavailability, an equivalent-qualified Psychologist will be substituted with prior client notification.
9. Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. In the absence of resolution, matters shall be referred to the competent courts of Tashkent City, Republic of Uzbekistan, in accordance with the Civil Code of Uzbekistan.
10. Any additional services beyond the scope described in this Quotation Estimate will be subject to a separate written agreement and supplementary quotation.
