- This Quotation Estimate is valid for sixty (60) days from the date of issue. After this period, all rates for Psychologist services may be subject to revision based on market conditions in Vietnam Ho Chi Minh City.
- All Psychologist services will be delivered in compliance with the Vietnamese Law on Mental Health (Law No. 12/2016/QH14) and the professional ethical standards of the Vietnam Association of Psychology.
- Payment terms: 40% advance payment upon acceptance of this Quotation Estimate, 30% upon completion of the first four therapy sessions, and the remaining 30% upon final delivery of all services and reports.
- Payments are to be made via bank transfer to the account of MindBridge Psychological Services, Bank for Foreign Trade of Vietnam (Vietcombank), Branch: Ho Chi Minh City. Account details will be provided upon contract execution.
- Any additional Psychologist sessions, extended evaluations, or on-site visits beyond the scope defined in this Quotation Estimate will be billed separately at the unit rates specified in the itemized table above.
- All client information, session notes, and diagnostic reports are held in strict confidence in accordance with Vietnamese data protection regulations. No information will be disclosed to third parties without written consent from the client, except where required by law in Vietnam Ho Chi Minh City.
- The Psychologist assigned to this engagement is Dr. Nguyen Thi Minh Anh, Ph.D., licensed under Registration No. 2019-PSY-0482 by the Ho Chi Minh City Department of Health. In the event of unavailability, a qualified substitute psychologist of equivalent credentials will be assigned with prior client notification.
- Cancellation policy: Sessions may be rescheduled up to 24 hours in advance at no charge. Cancellations within 24 hours or no-shows will be charged at 50% of the session fee.
- This Quotation Estimate does not constitute a binding contract. A formal service agreement will be executed upon mutual acceptance of all terms herein.
- All services are to be conducted within the administrative boundaries of Vietnam Ho Chi Minh City. Travel to locations outside the city (e.g., Binh Duong, Dong Nai) will incur additional transportation surcharges as per a separate schedule.
