The Radiologist proposed under this Quotation Estimate holds the following credentials: Board Certification in Diagnostic Radiology by the Brazilian College of Radiology (Colégio Brasileiro de Radiologia); Fellowship in Neuroradiology and Musculoskeletal Imaging; active registration with the Conselho Federal de Medicina (CFM) under CRM 123.456-SP; and a minimum of 12 years of clinical experience in tertiary-care hospitals within the São Paulo metropolitan region. The Radiologist is fully licensed to practice in the state of São Paulo, Brazil, and maintains malpractice insurance coverage of R$ 5,000,000.00 per occurrence through a nationally recognized insurer.
4. Terms and Conditions of This Quotation Estimate- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiration date, all prices listed herein are subject to revision based on prevailing market conditions in Brazil São Paulo.
- All prices are quoted in Brazilian Reais (BRL) and are inclusive of applicable taxes (ISS — Imposto Sobre Serviços) as mandated by the municipality of São Paulo. Any additional federal or state levies imposed after the date of this Quotation Estimate shall be borne by the client.
