Thank you for requesting a Quotation Estimate for the professional services of a certified Radiologist to be provided at your facility in Colombia Bogotá. MediRad Bogotá S.A.S. is pleased to present this detailed proposal for your review. This Quotation Estimate outlines the scope of radiological services, associated costs, and contractual terms for the engagement of a board-certified Radiologist to support diagnostic imaging operations at your clinic in the Bogotá metropolitan area.

The following Quotation Estimate covers the provision of a full-time, board-certified Radiologist (certified by the Colombian Medical Association – AMC and the Colombian Radiology Society – SOCIR) to perform diagnostic imaging interpretation, clinical consultation, and quality assurance services at your facility located in Bogotá, Colombia. The Radiologist will be available Monday through Friday, 8:00 AM to 5:00 PM, with optional Saturday coverage as detailed below.

Amount in words: Thirty million three hundred sixty thousand Colombian Pesos (COP $30,360,000.00). This Quotation Estimate is denominated in Colombian Pesos and is subject to the tax regulations of the National Tax Authority (DIAN) of Colombia.

Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing for the Radiologist services in Colombia Bogotá may be subject to revision based on market conditions and regulatory changes.
Payment Terms: Payment shall be made within fifteen (15) calendar days from the date of invoice issuance. Payment shall be made via bank transfer to the account of MediRad Bogotá S.A.S. at Bancolombia, Bogotá branch. Late payments shall accrue interest at the maximum rate permitted by Colombian law (Ley 1470 de 2011).
Contract Duration: The engagement of the Radiologist shall be for an initial term of twelve (12) months, renewable by mutual written agreement. Either party may terminate the contract with sixty (60) days prior written notice.
Regulatory Compliance: The Radiologist shall comply with all regulations established by the Colombian Ministry of Health and Social Welfare (Ministerio de Salud y Protección Social), the Superintendencia Nacional de Salud, and the Colombian Radiology Society (SOCIR). All diagnostic reports shall be issued in accordance with Colombian medical practice standards.
Confidentiality: All patient data and clinical information handled by the Radiologist in Bogotá shall be treated in strict accordance with Ley 1581 de 2012 (Colombian Data Protection Law) and applicable HIPAA-equivalent protocols.
Liability & Insurance: MediRad Bogotá S.A.S. shall maintain professional liability insurance (Seguro de Responsabilidad Civil Profesional) with a minimum coverage of COP 500,000,000 per claim, valid throughout the service period in Colombia.
Force Majeure: Neither party shall be liable for delays or non-performance resulting from events beyond reasonable control, including but not limited to natural disasters, government mandates, or public health emergencies as declared by the Colombian government.
Governing Law: This Quotation Estimate and any resulting contract shall be governed by the laws of the Republic of Colombia. Any disputes shall be resolved through the competent courts of Bogotá D.C., Colombia, or by arbitration before the Centro de Arbitraje y Conciliación de la Cámara de Comercio de Bogotá.
Scope Changes: Any modification to the scope of Radiologist services, additional imaging modalities, or extended coverage hours in Bogotá shall require a written amendment to this Quotation Estimate, signed by both parties.
