This Quotation Estimate is issued by Clinique Régionale d'Imagerie Médicale, a certified radiology practice operating in France Paris, in response to the formal request submitted by Hôpital Saint-Michel for the provision of comprehensive diagnostic imaging services. The undersigned Radiologist, Dr. Émile Vasseur, holds full certification from the Ordre des Médecins de France (ADELI registration) and is authorized to perform all radiological procedures listed herein in accordance with French medical regulations and the HAS (Haute Autorité de Santé) guidelines.
The services described in this Quotation Estimate encompass a full spectrum of radiological diagnostic work to be conducted at our facility in France Paris, with all imaging performed by a board-certified Radiologist and reported within the stipulated turnaround times. All equipment is CE-marked and maintained in compliance with the French Nuclear Safety Authority (ASN) regulations.

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on current French healthcare cost indices and supplier adjustments.
- All radiological services will be performed by a qualified Radiologist or a team of radiologists at our dedicated facility located in France Paris (75003, 3rd Arrondissement). The facility operates Monday through Saturday, 07:00 to 21:00, with 24/7 emergency coverage as specified in item 08.
- Payment terms: Net 45 days from the date of invoice issuance. Invoices will be issued monthly in arrears. Late payments will incur interest at the rate prescribed by Article L441-10 of the French Commercial Code.
- All diagnostic reports will be delivered electronically via the secure French health data platform (DMP — Dossier Médical Partagé) within 24 hours for standard studies and within 4 hours for urgent/emergency requests. A hard copy may be provided upon request at no additional cost.
- The Radiologist performing each procedure assumes full professional liability in accordance with French medical malpractice law. Professional indemnity insurance is maintained with a coverage limit of €5,000,000 per claim.
- Contrast media, radiopharmaceuticals, and all consumables are included in the unit prices listed above. No additional charges will be applied unless explicitly agreed in writing by both parties.
- This Quotation Estimate does not constitute a binding contract. A formal service agreement (Contrat de Prestation de Services) must be executed by both parties prior to the commencement of any radiological work.
- All patient data processed under this engagement shall comply with the French Data Protection Act (Loi Informatique et Libertés) and the European General Data Protection Regulation (GDPR). Data will be stored on servers physically located within France Paris metropolitan area.
- Any dispute arising from this Quotation Estimate or the subsequent service agreement shall be subject to the exclusive jurisdiction of the Tribunal de Commerce de Paris, France.
By signing below, both parties acknowledge that they have read, understood, and agree to the terms set forth in this Quotation Estimate for Radiologist diagnostic services to be delivered in France Paris. This document serves as the basis for the subsequent formal service contract.
