This Quotation Estimate is issued by MediScan Radiology & Diagnostic Centre Sdn. Bhd. to provide professional Radiologist services for the period of twelve (12) months, commencing 1 August 2025. - This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on prevailing market conditions in Malaysia Kuala Lumpur.
- All fees quoted are in Malaysian Ringgit (MYR) and are inclusive of the Radiologist's professional time, report generation, and standard administrative overheads. Any additional services not listed herein shall be subject to a separate written amendment.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate; the remaining 50% shall be settled in two equal monthly instalments within the contract period. Payment is due within fourteen (14) days of invoice date via bank transfer to the account specified on the invoice.
- The assigned Radiologist is a fully registered medical practitioner with the Malaysian Medical Council (MMC) and holds a valid specialist registration in Diagnostic Radiology. A copy of the registration certificate is available upon request.
- Confidentiality: All patient data, imaging studies, and radiological reports handled by the Radiologist in Malaysia Kuala Lumpur shall be treated in strict accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia and the facility's data governance policies.
- Liability: The Radiologist's professional liability is covered under a medical malpractice insurance policy with a minimum coverage of MYR 2,000,000 per claim, maintained throughout the contract period.
- Cancellation: Either party may terminate this agreement with sixty (60) days' written notice. In the event of early termination, fees for services already rendered shall remain payable.
- Governing Law: This Quotation Estimate and any resulting contract shall be governed by and construed in accordance with the laws of Malaysia. Any disputes shall be resolved through the courts of Kuala Lumpur, Federal Territory, Malaysia.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by authorised representatives.
By signing below, the Client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined for the Radiologist services to be provided in Malaysia Kuala Lumpur.
