This Quotation Estimate is issued by MediScan Imaging & Diagnostic Center to provide professional Radiologist services for the client identified above. The scope encompasses the full spectrum of diagnostic radiology interpretation, consultation, and reporting services to be delivered at our facility located in Mexico Mexico City, as well as on-site coverage at the client's premises within the same metropolitan area. The Radiologist assigned to this engagement will hold a valid license issued by the Consejo de Radiología y Protección Radiológica (CRRPR) and will possess a minimum of fifteen (15) years of clinical experience in diagnostic imaging, interventional radiology, and advanced modalities including MRI, CT, ultrasound, and digital mammography.

• Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing for Radiologist services may be subject to revision based on market conditions in Mexico Mexico City.
• Payment Terms: Net 30 days from the date of invoice issuance. Payment shall be made via bank transfer to the account designated by MediScan Imaging & Diagnostic Center, Mexico Mexico City. Late payments will accrue interest at the rate established by the Banco de México (Banxico) for overdue commercial obligations.
• Scope Limitations: The Radiologist services described herein do not include the cost of imaging equipment, contrast media, or consumables. These items are to be provided by the client facility.
• Licensing and Compliance: All Radiologist personnel assigned under this Quotation Estimate will maintain active professional licenses with the Secretaría de Salud and the Colegio de Médicos Radiólogos de México. Compliance with NOM-016-SSA3-2012 and applicable radiation safety regulations in Mexico Mexico City is guaranteed.
• Confidentiality: All patient data and imaging studies handled by the Radiologist team will be protected in accordance with the Ley Federal de Protección de Datos Personales en Posesión de los Particulares and institutional privacy policies.
• Termination: Either party may terminate the service agreement with sixty (60) days written notice. In the event of early termination, a pro-rated fee for the Radiologist services rendered up to the termination date will be invoiced.
• Dispute Resolution: Any disputes arising from this Quotation Estimate or the subsequent service contract shall be resolved through arbitration in Mexico Mexico City, in accordance with the Ley de Arbitraje Comercial de México.
• Force Majeure: Neither party shall be liable for delays or failures in performance caused by events beyond reasonable control, including but not limited to natural disasters, pandemics, or government-mandated closures in Mexico Mexico City.

By signing below, the client acknowledges receipt of this Quotation Estimate for Radiologist services and agrees to the terms outlined herein. This document does not constitute a binding contract until a formal service agreement is executed by both parties. MediScan Imaging & Diagnostic Center reserves the right to modify the scope of Radiologist services with prior written consent from the client.
