1. Purpose of This Quotation Estimate

This Quotation Estimate is issued by Dr. Aung Myo Min, a board-certified Radiologist with over fifteen years of clinical and diagnostic imaging experience, to provide a comprehensive and transparent cost breakdown for professional radiology services to be delivered at the Yangon Diagnostic Imaging Centre located in the heart of Myanmar Yangon. This document serves as a formal financial proposal outlining the scope of work, associated fees, and terms governing the engagement of the Radiologist for the requested period. All pricing reflects current market rates applicable to the Myanmar Yangon healthcare sector as of the date of issue.

2. Scope of Services

The Radiologist engaged under this Quotation Estimate shall provide the following diagnostic imaging and reporting services at the facility in Myanmar Yangon:

• Interpretation and written reporting of all X-ray (radiographic) examinations, including chest, abdominal, skeletal, and dental imaging.
• Ultrasound (sonographic) examinations covering obstetric, abdominal, pelvic, vascular, and small-parts imaging.
• Computed Tomography (CT) scan interpretation for head, chest, abdomen, pelvis, and whole-body protocols.
• Magnetic Resonance Imaging (MRI) interpretation for neurological, musculoskeletal, and soft-tissue studies.
• Fluoroscopic and contrast-enhanced studies, including upper GI, barium enema, and urographic procedures.
• Mammographic screening and diagnostic breast imaging with BI-RADS classification.
• Consultation with referring physicians regarding imaging findings and further diagnostic recommendations.
• Emergency and on-call radiology coverage as per the agreed schedule.

Note: All rates quoted in this Quotation Estimate are exclusive of applicable Myanmar government taxes, service charges, and any third-party laboratory or contrast-media costs. The Radiologist fees do not include the cost of imaging equipment, consumables, or facility overheads, which shall be borne separately by the client institution in Myanmar Yangon.

4. Terms and Conditions

1. This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, rates may be subject to revision based on prevailing economic conditions in Myanmar Yangon.
2. Payment shall be made on a monthly basis within fifteen (15) days of the end of each billing cycle. Late payments shall attract a penalty of 2% per month on the outstanding balance.
3. The Radiologist shall be available for clinical duties a minimum of five (5) days per week, with on-call coverage as specified in Item 07 of the fee schedule.
4. All diagnostic reports shall be delivered electronically within four (4) hours for routine studies and within one (1) hour for emergency or critical findings, in accordance with the standards of the Myanmar Medical Council.
5. The Radiologist shall maintain full professional indemnity insurance coverage throughout the duration of this engagement.
6. Either party may terminate this agreement with thirty (30) days written notice. In the event of early termination, all services rendered up to the termination date shall be invoiced in full.
7. All patient data and imaging records shall be handled in strict compliance with Myanmar's data protection regulations and the ethical standards of the Myanmar Radiological Society.
8. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by authorized representatives.
9. Any disputes arising from this engagement shall be resolved through amicable negotiation, and if unresolved, through arbitration in Myanmar Yangon in accordance with Myanmar law.

5. Acceptance and Authorization

By signing below, both parties acknowledge that they have read, understood, and agree to the terms set forth in this Quotation Estimate for Radiologist services to be provided in Myanmar Yangon.
