This Quotation Estimate is issued by MediRadiología del Pacífico S.A.C. to provide comprehensive Radiologist professional services to the above-named client, to be performed at the client's facility located in Peru Lima. The services encompass diagnostic imaging interpretation, clinical consultation, and ongoing radiological support as detailed below. All services will be delivered in accordance with the standards established by the Peruvian Ministry of Health (MINSA) and the Peruvian College of Radiologists (Colegio de Radiólogos del Perú).

The Radiologist assigned to this engagement, Dr. Elena Vargas Quispe (COFAM No. 91452, Board Certified in Diagnostic Radiology and Interventional Radiology), will provide full-time coverage for a period of twelve (12) months, with a minimum of 40 hours per week on-site at the client's facility in the San Isidro district of Peru Lima. The Radiologist will be responsible for the interpretation of all imaging modalities including X-ray, Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Ultrasound, and Mammography, as well as the supervision of radiology technicians and the issuance of formal diagnostic reports within 24 hours of image acquisition.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing may be subject to revision based on market conditions in Peru Lima and applicable Peruvian labor regulations.
• All prices are quoted in Peruvian Soles (PEN) and are inclusive of the Radiologist's professional fees, but exclusive of IGV, which is applied as shown above in compliance with Peruvian tax law (Ley del IGV, D.S. 055-99-EF).
• Payment terms: Net 30 days from the date of invoice issuance. Invoices will be issued on the last business day of each calendar month. Late payments will accrue interest at the rate established by the Peruvian Central Bank (BCR) for overdue commercial obligations.
• The Radiologist services described herein are subject to the availability of the assigned professional. In the event of illness or unforeseen absence, a qualified substitute Radiologist with equivalent credentials will be provided at no additional cost to the client.
• This Quotation Estimate does not constitute a binding contract. A formal service agreement must be executed by both parties prior to the commencement of services. The agreement will be governed by the laws of the Republic of Peru, and any disputes will be resolved in the competent courts of Peru Lima.
• The client is responsible for providing all necessary imaging equipment, PACS infrastructure, and a suitable workspace for the Radiologist at the facility in Peru Lima.
• Confidentiality: All patient data and diagnostic information handled by the Radiologist will be protected in accordance with Peruvian Law No. 29733 (Personal Data Protection Law) and the ethical code of the Peruvian College of Radiologists.
• Termination: Either party may terminate the service agreement with sixty (60) days' written notice. In the event of termination, the client shall pay for all services rendered up to the effective date of termination.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined herein for the provision of Radiologist services in Peru Lima. This acceptance does not obligate either party until a definitive service contract is formally executed.
