1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, all pricing is subject to revision based on prevailing market conditions in Senegal Dakar and applicable regulatory changes by the Senegalese Ministry of Health.
2. The Radiologist named in this document, Dr. Aïssatou Ndiaye, holds full professional licensure issued by the Ordre des Médecins du Sénégal and is registered with the national radiology board. All credentials shall be verified prior to commencement of services.
3. Payment terms: A deposit of twenty-five percent (25%) of the total Quotation Estimate amount is due upon acceptance. The remaining balance shall be invoiced monthly in arrears, with payment due within fifteen (15) business days of invoice receipt.
4. All services shall be performed in strict compliance with the radiological safety standards established by the Senegalese regulatory authority and international guidelines from the International Atomic Energy Agency (IAEA).
5. The Radiologist shall maintain professional medical malpractice insurance with a minimum coverage of 50,000,000 XOF, valid throughout the duration of this engagement in Senegal Dakar.
6. Any additional services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment and mutual agreement between both parties.
7. This agreement is governed by the laws of the Republic of Senegal. Any disputes arising from this Quotation Estimate shall be resolved through the competent courts of Dakar, Senegal.
8. The Radiologist is obligated to maintain patient confidentiality in accordance with Senegalese data protection legislation and the ethical standards of the medical profession.
Acceptance and AuthorizationBy signing below, both parties acknowledge and accept the terms, conditions, and pricing outlined in this Quotation Estimate for Radiologist services to be delivered in Senegal Dakar. This document constitutes a binding agreement upon countersignature by both authorized representatives.
