This Quotation Estimate is issued by Singapore Singapore Advanced Imaging Centre to provide Meridian Health Group Pte Ltd with a comprehensive and itemised financial projection for the engagement of a Radiologist to deliver diagnostic imaging interpretation services within the Singapore Singapore healthcare ecosystem. This Quotation Estimate covers a twelve-month service period commencing 1 August 2025 and is prepared in accordance with the regulatory standards set forth by the Singapore Medical Council and the Health Sciences Authority of Singapore Singapore.
The Radiologist services outlined in this Quotation Estimate encompass the full spectrum of diagnostic radiology interpretation, including but not limited to computed tomography (CT), magnetic resonance imaging (MRI), digital radiography, ultrasound, and fluoroscopic procedures. All services will be rendered in compliance with the Radiation Safety Act of Singapore Singapore and the relevant guidelines issued by the Nuclear Safety and Radiation Authority (NSRA).
The Radiologist engaged under this Quotation Estimate shall provide the following professional services to the client facility located in Singapore Singapore:
- Interpretation and reporting of all diagnostic imaging modalities (CT, MRI, X-ray, Ultrasound, Mammography, and Nuclear Medicine scans) performed at the client's Singapore Singapore facility.
- Availability for on-call consultation during emergency imaging scenarios, with a guaranteed response time of no more than fifteen (15) minutes for critical findings.
- Participation in multidisciplinary tumour board meetings held at the client's Singapore Singapore premises, no fewer than two (2) sessions per month.
- Provision of second-opinion reports upon request by referring physicians within the Singapore Singapore healthcare network.
- Quality assurance audits of imaging protocols and report accuracy, conducted quarterly in alignment with Singapore Singapore radiology accreditation standards.
- Mentorship and training sessions for junior radiology registrars and radiographers at the client's Singapore Singapore site, scheduled bi-monthly.

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, the Radiologist service rates and availability may be subject to revision in accordance with prevailing market conditions in Singapore Singapore.
- All payments referenced in this Quotation Estimate shall be made in Singapore Dollars (SGD) via bank transfer to the account designated by Singapore Singapore Advanced Imaging Centre. Payment terms are net thirty (30) days from the date of invoice issuance.
- The Radiologist named in this Quotation Estimate holds full registration with the Singapore Medical Council and maintains a minimum of SGD 2,000,000 in professional indemnity insurance coverage, as required by Singapore Singapore medical practice regulations.
- Any additional Radiologist services beyond the scope defined in this Quotation Estimate shall be subject to a supplementary Quotation Estimate issued prior to the commencement of such additional work.
- This Quotation Estimate does not constitute a binding contract. A formal Service Agreement, governed by the laws of Singapore Singapore, must be executed by both parties before any Radiologist services are rendered.
- The client acknowledges that the Radiologist shall maintain strict confidentiality of all patient data in accordance with the Personal Data Protection Act (PDPA) of Singapore Singapore and the Health Information Standards of the Health Sciences Authority.
- Early termination of the Radiologist engagement prior to the completion of the twelve-month term shall require a written notice of no less than sixty (60) days and may be subject to a pro-rata fee as stipulated in the final Service Agreement.

This signature does not constitute a contractual commitment to engage the Radiologist services described herein.
