- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period requires written reconfirmation from Madrid Advanced Radiology Centre, S.L.
- All prices stated in this Quotation Estimate are denominated in Euros (EUR) and are subject to the applicable Spanish Value Added Tax (Impuesto sobre el Valor Añadido — IVA) at the current rate of 21%, in accordance with the Ley 37/1992 del IVA.
- The Radiologist named in this document is a self-employed professional (autónomo) registered with the Spanish Social Security system (Seguridad Social) and the corresponding professional college in Madrid. All social security contributions and professional liability insurance (seguro de responsabilidad civil profesional) are the responsibility of the service provider.
- Payment terms: 50% upon written acceptance of this Quotation Estimate and 50% upon completion of the contracted service period. Invoices shall be issued in accordance with the Spanish electronic invoicing regulations (Factura Electrónica — Verifactu / SII).
- The Radiologist shall comply with all applicable Spanish medical regulations, including the Ley 41/2002 de Autonomía del Paciente, the professional code of ethics of the Consejo General de Colegios Oficiales de Médicos, and the specific regulations of the Comunidad de Madrid regarding diagnostic imaging centres.
- Confidentiality: All patient data processed by the Radiologist in the course of this engagement in Spain Madrid shall be treated in strict accordance with the GDPR and the LOPDGDD. A Data Processing Agreement (DPA) shall be executed prior to the commencement of services.
- Liability: The service provider maintains a professional liability insurance policy with a minimum coverage of 1,000,000 EUR per claim, issued by a Spanish-licensed insurer. A certificate of insurance shall be provided upon request.
- Termination: Either party may terminate the service agreement with thirty (30) days' written notice. In the event of early termination, the Radiologist shall be compensated for services rendered up to the termination date.
- Governing Law and Jurisdiction: This Quotation Estimate and any resulting contract shall be governed by the laws of Spain. Any disputes shall be submitted to the competent courts of Madrid, Spain, with express waiver of any other jurisdiction.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned. The acceptance shall be communicated to the service provider via registered mail or certified email to the address stated above.
By signing below, both parties acknowledge that they have read, understood, and agree to the terms set forth in this Quotation Estimate for the engagement of the Radiologist to provide diagnostic imaging services in Spain Madrid.
