This Quotation Estimate has been prepared in accordance with the formal request submitted by Sri Lanka General Hospital for the engagement of a qualified Radiologist to provide comprehensive diagnostic imaging interpretation services at their facility in Sri Lanka Colombo. This document outlines the scope of work, professional fees, equipment requirements, and all associated costs for a twelve-month service engagement period. The Quotation Estimate is structured to provide full transparency and aligns with the regulatory standards set forth by the Sri Lanka Medical Council and the Ministry of Health, Sri Lanka.
The Radiologist engaged under this Quotation Estimate shall provide the following professional services at the Colombo Diagnostic Imaging Centre and, as required, at the Sri Lanka General Hospital premises in Colombo:
- Interpretation and reporting of all X-ray, CT scan, MRI, ultrasound, and mammography images
- Emergency radiology consultation services available on a 24/7 basis
- Interventional radiology procedures including image-guided biopsies and drainage
- Monthly quality assurance audits of imaging equipment and reporting accuracy
- Training and mentorship sessions for junior radiology staff at the Colombo facility
- Participation in multidisciplinary tumor board meetings as required
- Issuance of second opinions and specialist referrals for complex cases

Important Terms Governing This Quotation Estimate
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Any acceptance beyond this period may be subject to revised pricing due to market fluctuations in Sri Lanka Colombo.
- Payment terms: 40% advance upon contract signing, 30% at the six-month milestone, and 30% upon successful completion of the twelve-month engagement. All payments shall be made in Sri Lankan Rupees (LKR) via bank transfer to the account specified in the service agreement.
- The Radiologist shall maintain full registration with the Sri Lanka Medical Council and hold a valid specialist certificate in Diagnostic and Interventional Radiology. Proof of credentials shall be provided prior to commencement of services.
- All patient data and imaging records shall be handled in strict compliance with the Sri Lanka Data Protection Act No. 9 of 2022. No patient information shall be transferred outside the premises of the Colombo facility without written consent.
- Either party may terminate this engagement with thirty (30) days written notice. In the event of early termination, a pro-rata adjustment shall be applied to the remaining fees as outlined in this Quotation Estimate.
- The Radiologist shall be available for a minimum of 40 hours per week at the designated Colombo facility, with an additional on-call commitment of 8 hours per week for emergency imaging interpretation.
- All equipment and software listed in this Quotation Estimate shall be installed, calibrated, and made operational within fourteen (14) days of contract execution.
- Disputes arising from this engagement shall be resolved through mediation in Colombo, Sri Lanka, in accordance with the laws of the Democratic Socialist Republic of Sri Lanka.
By signing below, both parties acknowledge that they have read, understood, and agree to all terms, conditions, and cost items detailed in this Quotation Estimate for Radiologist services to be delivered in Sri Lanka Colombo. This document, once countersigned, shall form an integral part of the master service agreement between the parties.
