This Quotation Estimate is issued by Kampala Advanced Radiology & Imaging Centre, a fully licensed and accredited diagnostic imaging facility operating in Uganda Kampala, in response to the formal request for professional Radiologist services. This document outlines the comprehensive scope of radiological services, associated professional fees, equipment utilization charges, and all applicable terms governing the engagement of our senior Radiologist team. All pricing in this Quotation Estimate is presented in Ugandan Shillings (UGX) and reflects current market rates for specialist radiological practice in Uganda Kampala as of the second quarter of 2025.

The following Radiologist services are included in this Quotation Estimate for the period of twelve (12) months, commencing upon mutual agreement and execution of the service contract. Our Radiologist team, comprising three (3) senior consultants and two (2) sub-specialists, will provide the following diagnostic and interventional radiology services at the client's facility in Uganda Kampala:

The following items are not included in the base Quotation Estimate above and will be billed separately if applicable during the service period:

• Consumable materials (contrast agents, biopsy needles, catheters) for interventional radiology procedures: UGX 450,000 – 2,800,000 per procedure, depending on the specific Radiologist-guided intervention performed.
• After-hours or weekend emergency call-outs beyond the standard 24/7 on-call arrangement: UGX 1,200,000 per additional call-out event.
• Travel and accommodation for the Radiologist team when attending off-site imaging facilities within the Uganda Kampala metropolitan area or surrounding districts: UGX 350,000 per day, inclusive of transport.
• Second-opinion or tertiary referral consultations with external specialist Radiologist partners: UGX 850,000 per consultation.

1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on prevailing market conditions in Uganda Kampala.
2. Payment terms: 40% advance payment upon contract signing, with the remaining 60% payable in two equal quarterly installments. All payments shall be made via bank transfer to the account designated by Kampala Advanced Radiology & Imaging Centre.
3. The Radiologist team shall comply with all data protection and patient confidentiality regulations as stipulated under the Uganda Data Protection and Privacy Act, 2019. All diagnostic reports and imaging data shall be stored securely and accessed only by authorized personnel.
4. Service delivery shall adhere to the minimum standards set by the Uganda Ministry of Health for diagnostic imaging facilities. The Radiologist on duty shall hold a valid UMDPC license and appropriate sub-specialty certification.
5. Either party may terminate the service agreement with a written notice of sixty (60) days. In the event of early termination, pro-rated charges for services already rendered shall be due in full.
6. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by authorized representatives.
7. All disputes arising from this engagement shall be resolved through mediation in Uganda Kampala, in accordance with the laws of the Republic of Uganda.

By signing below, the client acknowledges receipt of this Quotation Estimate for Radiologist services and agrees to the terms and conditions outlined herein. This acceptance authorizes Kampala Advanced Radiology & Imaging Centre to commence the provision of radiological services in Uganda Kampala as described in this document.
