- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiration date, all pricing and availability terms are subject to revision.
- All Radiologist services described in this document shall be performed by licensed medical professionals registered with the Medical Chamber of Uzbekistan and holding valid practice permits for the Tashkent region.
- Payment terms: 40% advance payment upon acceptance of this Quotation Estimate, 40% upon completion of six (6) months of service delivery, and 20% upon final completion of the full contract period.
- All diagnostic reports generated by the Radiologist shall be delivered in both English and Uzbek languages within twenty-four (24) hours of the examination, unless otherwise specified for emergency cases.
- The client acknowledges that all imaging procedures carry inherent risks. Informed consent forms, compliant with Uzbekistan healthcare legislation, shall be obtained prior to each procedure.
- MedScan Diagnostic Center shall maintain full medical malpractice insurance coverage for all Radiologist personnel operating in Tashkent, Uzbekistan, with a minimum coverage of $1,000,000 per incident.
- Any additional services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment signed by both parties.
- Disputes arising from this agreement shall be resolved through arbitration in Tashkent, Uzbekistan, in accordance with the Arbitration Law of the Republic of Uzbekistan.
- All patient data and medical records shall be handled in strict accordance with the Law of the Republic of Uzbekistan on Personal Data Protection and the confidentiality standards of the Tashkent Regional Health Department.
