1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. Rates are subject to revision upon expiration without prior notice.
2. All services rendered by the School Counselor shall comply with the Professional Regulation Commission (PRC) standards, the Code of Ethics for Psychologists in the Philippines, and the policies of the Department of Education (DepEd) Philippines Manila Division.
3. Payment terms: Net 30 days from the date of invoice issuance. Payment shall be made via bank transfer to the account of Educare Counseling Services Inc. at BDO Unibank, Makati City, Philippines Manila.
4. The School Counselor shall maintain strict confidentiality of all student records in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173). No student information shall be disclosed without written parental consent or a valid court order.
5. Cancellation policy: Either party may terminate the engagement with a thirty (30) day written notice. In the event of early termination by the client, a pro-rated fee for services already rendered shall be due.
6. The School Counselor shall not be held liable for outcomes beyond the scope of professional counseling services. In cases requiring psychiatric intervention, the School Counselor shall refer the student to a licensed psychiatrist in the Philippines Manila area.
7. This Quotation Estimate does not constitute a binding contract. A formal Service Agreement must be executed by both parties before services commence.
8. All rates are quoted in Philippine Pesos (PHP) and are inclusive of the School Counselor's professional fees, materials, and in-school transportation within the Philippines Manila metropolitan area.
