1. Validity: This Quotation Estimate remains valid for thirty (30) calendar days from the date of issuance. After the expiration date, pricing and availability of the School Counselor position may be subject to revision based on current labor market conditions in the United States Los Angeles area.
2. Payment Schedule: Invoices shall be issued on a monthly basis, due within thirty (30) days of receipt. A 1.5% late fee will be applied to any balance outstanding beyond the due date, in accordance with California Commercial Code Section 2202.
3. Scope of Services: The School Counselor engagement is limited to the services enumerated in this Quotation Estimate. Any additional programming, extended hours, or substitute coverage beyond the stated scope will require a written amendment and supplemental quotation.
4. Credentialing & Compliance: The assigned School Counselor shall hold a valid California Pupil Personnel Services (PPS) credential issued by the CTC. All background checks, fingerprinting, and clearance requirements mandated by the Los Angeles Unified School District and the State of California shall be completed prior to the first day of service.
5. Cancellation & Termination: Either party may terminate this agreement with sixty (60) days written notice. In the event of early termination by the client, a pro-rated charge for services rendered through the termination date will apply.
6. Confidentiality: All student records, session notes, and personal information handled by the School Counselor are protected under the Family Educational Rights and Privacy Act (FERPA) and California Education Code Section 49049. No data shall be disclosed without written parental or guardian consent.
7. Governing Law: This Quotation Estimate and any resulting contract shall be governed by the laws of the State of California, with jurisdiction residing in the Superior Court of Los Angeles County, United States.
8. Insurance: Los Angeles Educational Support Services, Inc. maintains general liability insurance of $2,000,000 per occurrence and professional liability (E&O) coverage of $1,000,000, with certificates available upon request.
