This Quotation Estimate is issued by Harmony Community Services Sdn. 4. Qualifications of the Social Worker

The Social Worker assigned to this engagement holds a Bachelor's or Master's degree in Social Work from a recognised Malaysian institution (e.g., Universiti Malaya, Universiti Sains Malaysia, or Universiti Teknologi MARA) and is registered with the Malaysian Social Workers Association (MSWA). The professional has a minimum of five (5) years of practising experience in the Malaysia Kuala Lumpur community and is proficient in Bahasa Malaysia, English, and Mandarin. All Social Worker engagements are conducted in strict compliance with the Code of Ethics of the Malaysian Social Workers Association and the Personal Data Protection Act 2010 (PDPA).

5. Payment Terms

Payment for this Quotation Estimate shall be made as follows: a non-refundable deposit of thirty percent (30%) of the total amount (RM 4,181.70) is due upon acceptance of this Quotation Estimate. The remaining seventy percent (70%) (RM 9,757.30) shall be payable in two equal instalments: fifty percent (50%) upon completion of the sixth (6th) counselling session, and the final fifty percent (50%) upon submission of the third and final progress report. All payments are to be made via bank transfer to the account of Harmony Community Services Sdn. Bhd. Payments are due within fourteen (14) calendar days of the invoice date. Late payments shall attract a penalty of two percent (2%) per month on the outstanding balance.

6. Terms and Conditions

This Quotation Estimate is valid for thirty (30) days from the date of issue. Beyond this period, all rates are subject to revision based on prevailing market conditions in Malaysia Kuala Lumpur.
The Social Worker reserves the right to withdraw from the engagement if the client fails to provide necessary cooperation, access, or documentation as reasonably required for the delivery of services.
All information shared during the course of the Social Worker engagement is subject to strict confidentiality as mandated by the Personal Data Protection Act 2010 and the professional ethical standards of the Malaysian social work profession.
This Quotation Estimate does not include costs for court appearances, psychiatric evaluations, or medical treatments, which shall be quoted separately if required.
Any additional services beyond the scope defined in Section 2 of this Quotation Estimate will be subject to a supplementary quotation approved in writing by both parties.
Disputes arising from this Quotation Estimate shall be resolved through mediation in Malaysia Kuala Lumpur in accordance with the Mediation Act 2012, and failing that, through the courts of competent jurisdiction in Malaysia Kuala Lumpur.
This Quotation Estimate is governed by the laws of Malaysia. The Social Worker services are provided in accordance with the Social Welfare Act 2010 and all applicable federal and state regulations in force at the time of service delivery.

7. Acceptance

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and rates set forth herein for the engagement of the Social Worker services in Malaysia Kuala Lumpur. This acceptance does not constitute a final contract; a separate Service Agreement will be executed within seven (7) business days of acceptance.
