3. Terms and Conditions of This Quotation Estimate- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, all rates and terms are subject to revision based on prevailing minimum wage adjustments in the Philippines Manila region and changes in the Social Worker professional fee schedule.
- Payment shall be made on a monthly basis, due on the 5th of each month, via bank transfer to the account designated by Cebu Pacific Social Services Inc. A 1.5% monthly surcharge shall apply to overdue payments.
- The assigned Social Worker shall report directly to the Barangay San Isidro Community Development Office and shall adhere to the Code of Ethics for Professional Social Workers in the Philippines as promulgated by the Philippine Association of Social Workers (PASW).
- All services shall be rendered within the geographic boundaries of the Philippines Manila metropolitan area, including but not limited to the cities of Manila, Quezon City, Makati, Pasig, and Mandaluyong. Travel beyond these boundaries shall be billed separately at a rate of PHP 150.00 per kilometer.
- The client reserves the right to request a replacement of the Social Worker with equivalent qualifications upon written notice of at least fifteen (15) days. No additional fee shall be charged for a single replacement during the contract term.
- Confidentiality of all client records, case files, and personal information handled by the Social Worker shall be maintained in strict accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) of the Philippines.
- This Quotation Estimate does not constitute a binding contract until both parties have executed a formal Service Agreement. Acceptance of this Quotation Estimate shall be indicated by the signature and date below.
- Any disputes arising from the services described in this Quotation Estimate shall be resolved through amicable negotiation, and failing that, through the appropriate courts of the Philippines Manila jurisdiction.
