Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. After the expiration date, all rates and availability of the assigned Social Worker are subject to reconfirmation.
Payment Terms: A 40% deposit ($3,323.42) is due upon acceptance of this Quotation Estimate to secure the Social Worker's schedule. The remaining 60% ($4,985.13) shall be invoiced in two equal monthly installments at the midpoint and conclusion of the service period.
Service Period: The estimated engagement period is twelve (12) weeks from the date of contract execution, subject to mutual agreement for extension.
Cancellation Policy: Cancellations with fewer than 48 hours' notice will incur a 50% fee of the scheduled session rate. No-shows will be billed at 100% of the applicable rate.
Licensing & Compliance: All Social Worker services are delivered in full compliance with the Florida Statutes Chapter 498, the Florida Board of Social Work regulations, and all applicable United States federal and state privacy laws, including HIPAA and 42 CFR Part 2.
Confidentiality: All client information, case files, and session records are maintained in strict confidence in accordance with professional ethical standards and Florida state law. Records are retained for a minimum of seven (7) years as required by the State of Florida.
Substitution: In the event the primary Social Worker is unavailable due to illness or leave, a qualified substitute LCSW of equivalent or higher credential will be assigned at no additional cost to the client.
Governing Law: This Quotation Estimate and any resulting contract shall be governed by the laws of the State of Florida, United States. Any disputes shall be resolved through mediation in Miami-Dade County before litigation is pursued.
