The following Speech Therapist services are included in this Quotation Estimate. Each service is delivered by a qualified and BSLT-registered Speech Therapist at our facility located in Banani, Bangladesh Dhaka, or at the client's designated location within the Bangladesh Dhaka city limits upon request.

The figures presented in this Quotation Estimate are based on a standard 12-month therapy engagement period. Should the client require an extended or abbreviated program, the Speech Therapist team will issue a revised Quotation Estimate at no additional charge. All rates are inclusive of VAT (5%) as mandated by the National Board of Revenue, Bangladesh Dhaka jurisdiction. No hidden fees, travel surcharges within Bangladesh Dhaka, or administrative costs are applied beyond what is itemized above.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, a new Quotation Estimate must be requested as rates for Speech Therapist services in Bangladesh Dhaka may be subject to revision.
• Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% payable in two equal monthly installments. All payments are to be made via bank transfer to the account of ClearVoice Speech & Language Therapy Centre, Dhaka Bank, Banani Branch, Bangladesh Dhaka.
• Sessions may be rescheduled up to 24 hours in advance without penalty. Cancellations made less than 24 hours prior to a scheduled session will incur a 50% session fee as charged by the assigned Speech Therapist.
• All therapy records, assessment reports, and progress documentation remain the property of the client. The Speech Therapist is bound by the Bangladesh Medical and Dental Council's code of ethics regarding patient confidentiality.
• This Quotation Estimate does not constitute a medical diagnosis. A formal diagnostic report will be provided following the initial assessment conducted by the senior Speech Therapist.
• Any services beyond the scope described in this Quotation Estimate will be billed separately at the prevailing Speech Therapist consultation rates in Bangladesh Dhaka.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms outlined herein for the engagement of Speech Therapist services provided by ClearVoice Speech & Language Therapy Centre, Bangladesh Dhaka.
