1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing may be subject to revision based on current operational costs in Canada Montreal.
2. A non-refundable deposit of 20% of the total estimated amount (CAD $5,357.00) is required to secure your client's place in the Speech Therapist's schedule. The remaining balance shall be paid in four (4) equal monthly installments of CAD $5,357.00, due on the 1st of each month.
3. All payments are accepted via Interac e-Transfer, credit card (Visa, Mastercard, Amex), or certified cheque made payable to Clarity Speech & Language Centre. No cash payments are accepted.
4. Sessions cancelled with less than 24 hours' notice will be charged at 50% of the session fee. Cancellations made 24 hours or more in advance will be rescheduled at no additional cost.
5. This Quotation Estimate does not constitute a fixed-price contract. The final invoice may vary by up to 10% depending on the actual number of sessions delivered, additional assessments required, or changes in the therapeutic plan as determined by the Speech Therapist.
6. Our Speech Therapist services in Canada Montreal are partially covered by certain extended health insurance plans and private insurance providers. We will provide you with detailed receipts suitable for insurance reimbursement upon request. Please verify coverage with your provider prior to commencing treatment.
7. All services are delivered in compliance with the Professional Code of Ethics of the Quebec College of Speech-Language Pathologists and Audiologists. Confidentiality of all client records is maintained in accordance with Quebec's Law 25 (Act respecting the protection of personal information in the private sector) and federal privacy legislation.
8. The Speech Therapist reserves the right to modify the treatment plan based on ongoing clinical observations. Any significant changes to the scope of services will be communicated in writing and a revised Quotation Estimate will be provided for your approval before additional charges are incurred.
Additional NotesThis Quotation Estimate reflects a 16-week therapeutic program. The Speech Therapist will conduct a formal re-evaluation at the 8-week and 16-week marks to determine whether the program should be extended, modified, or concluded. Should additional weeks be required, the per-session rates outlined in this document will apply, and a supplementary Quotation Estimate will be issued. Our clinic in Canada Montreal operates Monday through Friday, 8:00 AM to 6:30 PM, and Saturday mornings by appointment. Telehealth sessions are available seven days per week.
