The Speech Therapist assigned to this case will follow an evidence-based, individualised treatment plan developed after the initial assessment. The intervention will be conducted at our dedicated clinic in Koramangala, India Bangalore, which is equipped with sound-attenuated therapy rooms, audiometric testing facilities, and a sensory-integration play area. All sessions will be documented in a secure digital case-management system, and the parent will receive a monthly progress summary. 1. A 25% advance payment of ₹ 73,160 (inclusive of GST) is due upon acceptance of this Quotation Estimate to reserve the Speech Therapist's schedule.
2. The remaining balance shall be payable in four equal monthly instalments of ₹ 54,870, due on the 5th of each month.
3. Accepted payment modes: NEFT/RTGS, UPI, credit/debit card, and bank transfer to the account details provided in the proforma invoice.
4. A late payment surcharge of 2% per month will apply to any overdue instalment.
1. This Quotation Estimate is valid for 30 days from the date of issue. Prices are subject to revision after the validity period due to changes in operational costs in India Bangalore.
2. The Speech Therapist reserves the right to modify the treatment plan based on the child's progress, with prior written consent from the parent/guardian.
3. Cancellation of a scheduled session must be communicated at least 24 hours in advance; otherwise, the session fee will be charged.
4. All clinical records and assessment reports remain the property of ClearVoice Speech & Language Centre and will be shared with the client upon request.
5. This estimate does not include costs for medical diagnostics (audiometry, ENT consultation, or imaging) that may be recommended by the Speech Therapist during the course of treatment.
6. Disputes arising from this Quotation Estimate shall be governed by the laws of India and subject to the exclusive jurisdiction of courts in India Bangalore.
