This Quotation Estimate is payable in three installments: 30% upon acceptance of this document, 40% at the midpoint of the therapy program (Week 6), and the remaining 30% upon completion of the final session. Payment may be made via bank transfer to Shinagawa Wellness & Rehabilitation Center's account at MUFG Bank, Koto Branch, Tokyo, Japan, or by credit card (Visa, Mastercard, JCB). All payments are denominated in Japanese Yen (JPY). In the event that the client holds valid Japan Tokyo health insurance coverage (National Health Insurance or private supplementary insurance), the applicable reimbursement portion will be deducted, and the client will be responsible only for the co-payment amount as determined by the insurer.
4.1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, pricing may be subject to revision based on current operational costs in Japan Tokyo.
4.2. The Speech Therapist reserves the right to modify the therapy plan based on clinical progress. Any additional sessions beyond the 24 individual and 8 group sessions outlined above will be billed at the standard per-session rate of ¥18,500 and ¥12,000 respectively, subject to prior written approval by the client.
4.3. Cancellations must be made at least 48 hours in advance. Sessions cancelled with less than 48 hours' notice will be charged at 50% of the session fee. No-shows will be charged in full.
4.4. All clinical records, assessment reports, and therapy documentation are maintained in strict compliance with Japan's Act on the Protection of Personal Information (APPI) and the Tokyo Metropolitan Medical Association's confidentiality guidelines.
4.5. The Speech Therapist will provide a written progress summary to the referring physician every four weeks and a comprehensive final report upon program completion.
4.6. This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Speech Therapist will employ evidence-based methodologies, but individual results may vary based on the client's condition, adherence to home practice, and overall health.
4.7. In the event of force majeure (natural disasters, public health emergencies affecting Japan Tokyo, or government-mandated facility closures), sessions will be rescheduled at no additional cost to the client.
By signing below, the client (or legal guardian) acknowledges receipt of this Quotation Estimate, agrees to the terms and conditions outlined herein, and authorizes Shinagawa Wellness & Rehabilitation Center to commence the Speech Therapist program in Japan Tokyo as described above.
