This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. All prices are quoted in Kazakhstani Tenge (KZT) and are inclusive of applicable Value Added Tax (VAT) at the current Kazakhstan rate of 12%. Payment terms are structured as follows: 30% advance payment upon acceptance of this Quotation Estimate, 40% upon completion of the first six months of service delivery, and the remaining 30% upon final project completion and submission of the comprehensive annual report.
The Speech Therapist assigned to this engagement holds a Master's degree in Speech-Language Pathology, is licensed by the Ministry of Health of the Republic of Kazakhstan, and possesses a minimum of eight years of clinical experience in the Kazakhstan Almaty region. The Speech Therapist will maintain full professional liability insurance throughout the duration of the contract. Any changes to the scope of services, patient volume, or session frequency must be documented in a written amendment to this Quotation Estimate and signed by both parties before additional work commences.
Confidentiality of all patient records and clinical data is guaranteed in full compliance with the Law of the Republic of Kazakhstan "On Personal Data and Its Protection."
