1. The purpose of this document is to ensure full transparency regarding the scope of therapy, associated costs, session schedules, and all applicable terms governing the engagement. 2. Scope of Speech Therapist ServicesThe following Speech Therapist interventions have been recommended following the initial diagnostic assessment conducted on 28 May 2025 at our Myanmar Yangon clinic. The treatment plan addresses articulation disorders, phonological processing difficulties, and mild expressive language delays identified in the client's 7-year-old dependent, Thet Htet Aung.

All Speech Therapist professionals assigned to this engagement in Myanmar Yangon hold a minimum of a Master's degree in Speech-Language Pathology from an accredited institution. The lead Speech Therapist, Dr. Thiri Aung, is registered with the Myanmar Medical Council and holds additional certification in pediatric speech disorders from the American Speech-Language-Hearing Association (ASHA). All supporting therapists are licensed practitioners with a minimum of five years of clinical experience in the Myanmar Yangon healthcare setting.
4. Terms & Conditions of This Quotation Estimate4.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing for Speech Therapist services in Myanmar Yangon may be subject to revision based on operational cost adjustments.
4.2 Payment Schedule: A 30% deposit (MMK 4,488,000) is due upon acceptance of this Quotation Estimate. The remaining balance shall be payable in three equal monthly installments of MMK 3,490,667, due on the 1st of each month. Payments may be made via bank transfer to Yangon Bank, Account No. 001-2345-6789, or in cash at our Myanmar Yangon office.
4.3 Cancellation Policy: Sessions cancelled with less than 24 hours' notice will be charged at 50% of the session fee. The Speech Therapist reserves the right to reschedule sessions due to medical emergencies or public holidays as declared in Myanmar Yangon.
4.4 Scope Limitations: This Quotation Estimate covers the 3-month therapy programme as outlined. Any additional diagnostic testing, extended therapy beyond the initial plan, or specialist referrals will be quoted separately in a supplementary Quotation Estimate.
4.5 Confidentiality: All patient records, assessment data, and therapy progress notes maintained by the Speech Therapist team in Myanmar Yangon are protected under the Myanmar Personal Data Protection Law (2021). No information will be disclosed to third parties without written consent.
4.6 Governing Law: This agreement is governed by the laws of the Republic of the Union of Myanmar. Any disputes arising from this Quotation Estimate shall be resolved through mediation in Myanmar Yangon before proceeding to litigation.
5. Acceptance & AuthorizationBy signing below, the client acknowledges receipt of this Quotation Estimate and authorizes Yangon Speech & Communication Centre to commence the Speech Therapist services as described herein, to be delivered at our facility in Myanmar Yangon or at the client's residence as mutually agreed.
