The Speech Therapist assigned to this case, Ms. Anisha Gurung (Registered Speech-Language Pathologist, License No. SLT-NPL-2019-0342), holds a Master's degree in Speech-Language Pathology from Tribhuvan University, Kathmandu, and has completed additional certifications in pediatric speech disorders, aphasia rehabilitation, and articulation therapy. All services described in this Quotation Estimate will be conducted in a fully equipped therapy room at our Kathmandu facility, adhering to the clinical standards set by the Nepal Health Research Council and the Nepal Speech-Language-Hearing Association.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, the Speech Therapist service rates may be subject to revision based on prevailing market conditions in Nepal Kathmandu.
• Payment Terms: A 50% advance payment (NPR 46,050) is required to confirm the booking and reserve the Speech Therapist's schedule. The remaining 50% (NPR 45,550) shall be payable upon completion of the final session or within seven (7) days of the final assessment, whichever is earlier.
• Accepted payment methods include bank transfer (Nabil Bank, Rastriya Banijya Bank), mobile banking (eSewa, Khalti), and cash at our Kathmandu office. All transactions will be documented with official receipts.
• Rescheduling: The client may reschedule any individual Speech Therapist session up to 24 hours in advance without additional charge. Cancellations made less than 24 hours prior will be charged at 50% of the session fee.
• Missed sessions: If a session is missed without prior notice, it will not be rescheduled at no cost. Up to two (2) complimentary reschedules are permitted during the entire therapy period.
• All diagnostic reports, therapy notes, and progress documentation prepared by the Speech Therapist are the intellectual property of the client and will be handed over in both digital (PDF) and printed formats upon completion.
• This Quotation Estimate does not include costs for medical imaging (MRI, CT scan), audiological testing, or psychiatric consultation, should the Speech Therapist recommend additional specialist referrals during the course of treatment.
• Confidentiality: All patient information is protected under the Nepal Privacy Act, 2075 (2018). The Speech Therapist and all centre staff are bound by strict non-disclosure agreements.
• Any dispute arising from the services described in this Quotation Estimate shall be resolved through amicable negotiation, and if unresolved, through the competent courts in Kathmandu, Nepal.
