This Quotation Estimate is based on the following assumptions: (a) the client will provide all necessary raw data in machine-readable format within ten (10) business days of contract execution; (b) the Statistician will have unrestricted access to the client's secure data environment located in the United States Chicago office; (c) no more than two (2) major scope revisions will be requested during the engagement period. Costs associated with additional data collection, third-party data licensing beyond what is listed in Item 9, or travel to locations outside the United States Chicago metropolitan area (defined as Cook, DuPage, Lake, and Will counties) are excluded from this estimate and will be billed separately at prevailing rates.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, all rates and availability of the designated Statistician are subject to re-evaluation.
- Payment terms: 40% upon execution of the service agreement, 35% upon completion of Items 1–5, and 25% upon final delivery of all reports and documentation. All payments are due net thirty (30) days from invoice date.
- The Statistician engaged under this estimate holds a Ph.D. in Biostatistics from the University of Chicago and possesses over fourteen (14) years of professional experience in healthcare analytics within the United States Chicago region.
- All work product, including models, code, reports, and analytical outputs, shall become the exclusive property of the client upon full payment. The Statistician retains the right to reference the engagement in professional portfolios with client written consent.
- Confidentiality: All data and proprietary information shared during the engagement will be handled in strict accordance with HIPAA regulations, Illinois Personal Information Protection Act (815 ILCS 530), and the mutual Non-Disclosure Agreement executed concurrently with this Quotation Estimate.
- Force Majeure: Neither party shall be liable for delays caused by events beyond reasonable control, including but not limited to natural disasters affecting the United States Chicago area, pandemics, or governmental mandates.
- Governing Law: This agreement and this Quotation Estimate shall be governed by and construed in accordance with the laws of the State of Illinois, with venue in the Circuit Court of Cook County, United States Chicago.
