The purpose of this document is to provide a comprehensive, itemized financial estimate for the recruitment, deployment, and sustained operational support of a qualified Surgeon to serve at the National Trauma Center in Afghanistan Kabul. The Surgeon engagement described in this Quotation Estimate encompasses full-time clinical duties, on-call emergency response, surgical training of local medical staff, and participation in public health initiatives coordinated by the Ministry. The scope is designed to ensure continuity of surgical care in a region where access to specialist medical professionals remains critically limited.
- General & Trauma Surgery: Performance of all levels of general surgical procedures including laparotomy, hernia repair, appendectomy, cholecystectomy, and trauma-related interventions at the Afghanistan Kabul National Trauma Center.
- Emergency & On-Call Coverage: Availability for 24/7 emergency surgical response, including a minimum of 48 hours per week on-site presence and 72 hours per month on-call duty.
- Capacity Building & Mentorship: Supervision and structured training of a minimum of six (6) local surgical residents and two (2) anesthesiologists per quarter, conducted within the Afghanistan Kabul facility.

4.1 Payment Schedule: The total amount specified in this Quotation Estimate shall be disbursed in quarterly installments of 25% each, payable within thirty (30) days of the end of each calendar quarter. The first installment is due upon successful completion of the Surgeon's medical credentialing and security clearance in Afghanistan Kabul.
4.2 Validity: This Quotation Estimate remains valid for thirty (30) calendar days from the date of issue. Should the client require a revised estimate beyond this period, all costs shall be re-evaluated based on prevailing market conditions in Afghanistan Kabul.
4.3 Surgeon Qualifications: The Surgeon deployed under this agreement shall hold a minimum of a Doctor of Medicine (MD) degree, a recognized surgical fellowship (FRCS or equivalent), and a minimum of eight (8) years of post-qualification surgical experience, with at least three (3) years in trauma or general surgery. All credentials shall be verified by the Ministry prior to deployment.
4.4 Termination Clause: Either party may terminate this engagement with sixty (60) days written notice. In the event of force majeure, including but not limited to security deterioration in Afghanistan Kabul, both parties shall negotiate in good faith for a temporary suspension or relocation of the Surgeon's duties.
4.5 Confidentiality: All patient records, clinical data, and operational details pertaining to the Surgeon's work in Afghanistan Kabul shall be treated as strictly confidential and shall not be disclosed to third parties without written consent from the Ministry of Public Health.
4.6 Governing Law: This Quotation Estimate and the resulting contract shall be governed by and interpreted in accordance with the laws of the Islamic Republic of Afghanistan, with disputes subject to arbitration in Kabul.
