- This Quotation Estimate constitutes a non-binding proposal for the engagement of a Surgeon and does not constitute a contract until formally accepted in writing by both parties.
- All Surgeon services will be performed in strict compliance with the standards set by the College of Physicians and Surgeons of British Columbia and applicable federal and provincial health regulations in Canada Vancouver.
- Payment terms: 50% deposit due upon acceptance of this Quotation Estimate; remaining 50% due within fourteen (14) calendar days of procedure completion.
- The Surgeon reserves the right to decline or modify the surgical plan if, during the pre-operative assessment, the patient's condition warrants a different approach. Any such modifications will be communicated to the client prior to the procedure.
- All patient data and medical records handled by the Surgeon are protected under the Personal Information Protection Act (PIPA) of British Columbia and the federal Privacy Act of Canada.
- This Quotation Estimate does not include the cost of surgical instruments, implants, pharmaceuticals, or hospital/clinic facility fees, which will be billed separately by the respective providers in Canada Vancouver.
- Any cancellation of the Surgeon engagement by the client with less than seventy-two (72) hours' notice will incur a 25% cancellation fee on the total estimated amount.
- Disputes arising from this Quotation Estimate or the subsequent Surgeon engagement shall be resolved through mediation in accordance with the laws of the Province of British Columbia, Canada Vancouver jurisdiction.
By signing below, the client acknowledges receipt of this Quotation Estimate and authorizes the engagement of the Surgeon as described herein for services to be rendered in Canada Vancouver.
