3.1 This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, all pricing is subject to revision based on prevailing market conditions in China Shanghai and applicable regulatory adjustments by the National Health Commission.
3.2 Payment shall be made in Renminbi (CNY) via bank transfer to the designated account of Shanghai Elite Medical Group Co., Ltd. A 30% advance payment is required upon acceptance of this Quotation Estimate, with the remaining 70% disbursed in quarterly installments aligned with the Surgeon's service delivery milestones.
3.3 The Surgeon engaged under this agreement shall comply with all medical practice regulations, patient confidentiality laws (including the Personal Information Protection Law of the PRC), and institutional protocols of the Shanghai International Medical Center. Any breach of professional conduct will result in immediate termination and forfeiture of remaining compensation.
3.4 All surgical procedures performed by the Surgeon in China Shanghai shall be documented in the hospital's electronic medical record system and subject to quarterly peer review by the Medical Quality Assurance Committee.
3.5 This Quotation Estimate does not constitute a final contract. A formal Service Agreement, governed by the Civil Code of the People's Republic of China, shall be executed within fourteen (14) days of mutual acceptance of this document.
3.6 Any disputes arising from the interpretation or execution of this Quotation Estimate shall be resolved through arbitration at the Shanghai International Arbitration Center (SHIAC) in accordance with its then-current rules.
Note: All figures in this Quotation Estimate are exclusive of Value Added Tax (VAT) at the prevailing rate of 6% for medical services in China Shanghai. The final invoiced amount shall include applicable VAT and any government levies mandated by the Shanghai Municipal Tax Bureau. The Surgeon's personal income tax shall be withheld and remitted in accordance with the Individual Income Tax Law of the PRC.By signing below, both parties acknowledge that they have read, understood, and agree to the terms set forth in this Quotation Estimate for the engagement of the Surgeon in China Shanghai.
