The Surgeon engaged under this Quotation Estimate shall be a board-certified general surgeon with a minimum of twelve (12) years of post-qualification experience in general surgery, laparoscopic procedures, and emergency surgical interventions. The Surgeon shall be fully licensed and registered with the Kuwait Ministry of Health and shall hold a valid practice permit for Kuwait Kuwait City. The scope of services includes, but is not limited to, the following:

• Performance of elective and emergency general surgical procedures (appendectomy, cholecystectomy, hernia repair, bowel resection, and related operations).
• Minimally invasive and laparoscopic surgical interventions as clinically indicated.
• Pre-operative patient assessment, surgical planning, and post-operative follow-up care.
• On-call availability for emergency surgical cases within the hospital premises in Kuwait Kuwait City.
• Participation in multidisciplinary surgical case conferences and quality assurance committees.
• Mentorship and supervision of resident surgeons and surgical trainees as required by hospital policy.
• Maintenance of complete and accurate surgical documentation in the hospital's electronic medical records system.

• This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period shall require revalidation of all rates and terms.
• The Surgeon engagement is for an initial contract term of twelve (12) months, renewable upon mutual written agreement of both parties. The terms of this Quotation Estimate shall form the financial annex to the master service agreement.
• All payments shall be made in Kuwaiti Dinars (KWD) via bank transfer to the account designated by Al-Salem Medical Consultancy Group. Monthly professional fees are payable within fifteen (15) business days of the end of each calendar month.
• The Surgeon shall be subject to the labor laws of the State of Kuwait and the regulatory oversight of the Kuwait Ministry of Health. All practice activities shall be conducted within the licensed facility in Kuwait Kuwait City.
• Any surgical procedures exceeding the standard scope outlined in Section 2 shall be subject to a separate supplementary Quotation Estimate and prior written approval from the client's Chief Medical Officer.
• Early termination of the Surgeon engagement by either party prior to the completion of the initial twelve-month term shall incur a penalty equal to three (3) months of the base professional fee, as stipulated in the master agreement.
• This Quotation Estimate does not constitute a final contract. A definitive service agreement, incorporating all terms herein, shall be executed by both parties prior to the commencement of the Surgeon's duties.
• All disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved through arbitration in accordance with the Kuwait Commercial Arbitration Law, with the seat of arbitration in Kuwait Kuwait City.

By signing below, the client acknowledges receipt of this Quotation Estimate and confirms that the financial projections, scope of Surgeon services, and associated terms are acceptable for the purpose of initiating the contractual engagement in Kuwait Kuwait City.
