1. Purpose of This Quotation EstimateThis Quotation Estimate is issued by MediSurge Nepal Pvt. Ltd. in response to the formal request received from Shree Himalaya General Hospital, Nepal Kathmandu, for the engagement of a qualified and experienced Surgeon to provide specialized surgical services on a contractual basis. This document outlines the comprehensive scope of services, associated costs, and contractual terms governing the provision of Surgeon services within the Nepal Kathmandu metropolitan area. All figures presented in this Quotation Estimate are denominated in Nepalese Rupees (NPR) unless otherwise stated.
2. Scope of Surgeon ServicesThe Surgeon engaged under this Quotation Estimate shall provide the following services at the client's facility located in Nepal Kathmandu:
- General surgical procedures including laparotomy, hernia repair, appendectomy, and cholecystectomy.
- Emergency surgical interventions available on a 24/7 on-call basis within the Nepal Kathmandu district.
- Pre-operative patient assessment, surgical planning, and post-operative follow-up care.
- Participation in multidisciplinary surgical teams for complex cases requiring co-management.
- Mentorship and training of junior surgical residents at the Nepal Kathmandu facility.
- Monthly surgical audit reports and quality assurance documentation.

The Surgeon proposed under this Quotation Estimate shall meet the following minimum qualifications as mandated by the Nepal Medical Council and the Department of Health Services, Nepal Kathmandu:
- MBBS degree from a recognized medical university in Nepal or an equivalent foreign institution.
- Membership of the Nepal Medical Council (NMC) with a valid practicing license.
- Postgraduate qualification in General Surgery (MS/MD) or equivalent.
- Minimum 10 years of post-qualification surgical experience, with at least 5 years in a tertiary care hospital in Nepal Kathmandu or an internationally recognized surgical center.
- Valid professional indemnity insurance with a minimum coverage of NPR 10,000,000 per claim.
- Fluency in Nepali and English for patient communication and medical documentation.
- 30% advance payment upon acceptance of this Quotation Estimate and execution of the service agreement.
- Remaining 70% payable in equal monthly installments over the 12-month contract period.
- Specialized surgical procedure fees (Item 3) shall be invoiced per case within 7 working days of procedure completion.
- All payments shall be made via bank transfer to the account designated by MediSurge Nepal Pvt. Ltd., Nepal Kathmandu.
- Late payments shall attract an interest charge of 1.5% per month as per Nepal banking regulations.
- This Quotation Estimate is valid for 30 calendar days from the date of issue. Beyond this period, all rates are subject to revision based on prevailing market conditions in Nepal Kathmandu.
- The Surgeon shall comply with all applicable laws, regulations, and ethical guidelines of the Nepal Medical Council and the Ministry of Health and Population, Government of Nepal.
- Either party may terminate the engagement with 60 days' written notice. In the event of termination, a pro-rata adjustment shall be applied to the remaining contract value.
- All surgical records, patient data, and medical documentation generated during the engagement shall remain the property of the client hospital in Nepal Kathmandu.
- MediSurge Nepal Pvt. Ltd. shall not be held liable for surgical outcomes beyond the standard of care expected of a reasonably competent Surgeon in Nepal Kathmandu.
- Disputes arising from this Quotation Estimate or the resulting service agreement shall be resolved through arbitration in Nepal Kathmandu in accordance with the Arbitration Act of Nepal, 2055 (1998).
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined herein for the engagement of a Surgeon in Nepal Kathmandu.
