- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Upon expiration, all rates and availability of the Surgeon in Russia Moscow are subject to revision.
- Payment terms: Fifty percent (50%) of the Grand Total is due as a non-refundable deposit upon acceptance of this Quotation Estimate. The remaining fifty percent (50%) is payable within seven (7) business days following the completion of the primary surgical procedure.
- The Surgeon reserves the right to reschedule the procedure with a minimum of forty-eight (48) hours' written notice in the event of a medical emergency or force majeure circumstance within Moscow.
- All surgical services shall be performed in strict compliance with the Federal Law of the Russian Federation No. 323-FZ "On the Fundamentals of Health Protection of Citizens in the Russian Federation" and applicable Moscow municipal health regulations.
- The client acknowledges that surgical procedures carry inherent risks. A separate informed consent form, prepared in accordance with Russian medical law, must be signed by the patient prior to any operative intervention.
- This Quotation Estimate does not constitute a binding contract until countersigned by both parties. Acceptance is confirmed by written signature and the payment of the initial deposit.
- Confidentiality: All patient medical records and data generated during the engagement of the Surgeon in Russia Moscow shall be handled in accordance with Federal Law No. 152-FZ on Personal Data.
