The Surgeon proposed for this engagement is Dr. Emmanuel K. Mushi, a Board-Certified General and Vascular Surgeon with over 18 years of clinical experience in Tanzania Dar es Salaam and the broader East African region. Dr. Mushi holds a Fellowship of the Royal College of Surgeons (FRCS) and is registered with the Medical Board of Tanzania (MBT) under Practising Certificate No. MBT/2007/2234. The scope of services covered under this Quotation Estimate includes, but is not limited to:

• General surgical procedures (appendectomy, cholecystectomy, hernia repair, and bowel resection) performed at the client's operating theatre in Tanzania Dar es Salaam.
• Vascular and peripheral surgical interventions, including carotid endarterectomy and peripheral arterial bypass grafting.
• Pre-operative patient assessment, anaesthetic consultation coordination, and surgical planning sessions.
• Intra-operative leadership and direct surgical execution, including the use of a dedicated surgical team (scrub nurse, anaesthetist, and surgical assistant).
• Post-operative ward rounds, wound management, suture removal, and follow-up consultations for a minimum of 14 days per patient.
• Emergency surgical call-out availability (24/7) for critical cases at the Tanzania Dar es Salaam facility.
• Monthly clinical audit reports and surgical outcome documentation submitted to the hospital administration.

The total amount stated in this Quotation Estimate shall be payable in accordance with the following schedule:

• Advance Payment (30%): TZS 104,500,800 – payable within 14 calendar days of contract signature, prior to the commencement of the Surgeon's engagement.
• Quarterly Instalments (20% each): TZS 69,667,200 per quarter – payable within 10 business days of the end of each calendar quarter (March, June, September, December).
• Final Settlement (10%): TZS 34,833,600 – payable within 30 days of the completion of the 12-month contract period, subject to satisfactory delivery of all services and submission of the final clinical audit report.

All payments shall be made via bank transfer to the account of East African Surgical & Medical Services Ltd., National Bank of Commerce, Tanzania Dar es Salaam Branch, Account No. 0045-8821-3367. Invoices will be issued in accordance with Tanzania Revenue Authority (TRA) regulations.

• This Quotation Estimate is valid for a period of 30 calendar days from the date of issue. Beyond this period, all prices are subject to revision based on prevailing market conditions in Tanzania Dar es Salaam.
• The Surgeon shall comply with all clinical guidelines issued by the Tanzania Medical Association (TMA) and the Tanzania Food and Drug Authority (TFDA) throughout the duration of the engagement.
• The client (KCMC Hospital) shall provide a fully equipped operating theatre, anaesthetic support, and all necessary hospital infrastructure at the Tanzania Dar es Salaam facility. Any additional equipment not listed in this quotation shall be at the client's expense.
• Either party may terminate the contract with 60 days' written notice. In the event of early termination, the Surgeon's professional fee shall be pro-rated for the months actually served.
• All patient records, surgical reports, and clinical data generated during the engagement shall remain the property of KCMC Hospital, Tanzania Dar es Salaam, in compliance with the Tanzania Data Protection Act No. 6 of 2022.
• Any disputes arising from this Quotation Estimate or the resulting contract shall be resolved through mediation in Dar es Salaam, Tanzania, in accordance with the Arbitration Act of Tanzania.
• This quotation does not constitute a binding contract until a formal service agreement is executed by both parties.
