2. Scope of Surgeon ServicesThe following Surgeon services are included in this Quotation Estimate for delivery in Thailand Bangkok. The Surgeon shall be a board-certified specialist with a minimum of ten (10) years of post-graduate surgical experience, holding valid registration with the Medical Council of Thailand and appropriate specialist certification recognized by the Thai Ministry of Public Health. All services will be rendered at the designated medical facility located in the Pathum Wan district of Thailand Bangkok, or at an alternative site mutually agreed upon by both parties.

3.1 Validity: This Quotation Estimate shall remain valid for a period of thirty (30) calendar days from the date of issue. Any engagement initiated after the expiry date will require a revised Quotation Estimate reflecting current market rates for Surgeon services in Thailand Bangkok.
3.2 Payment Terms: A deposit of fifty percent (50%) of the total Quotation Estimate amount is due upon acceptance of this document. The remaining fifty percent (50%) shall be settled within fourteen (14) days following the completion of the final Surgeon service delivery or the end of the contracted retainer period, whichever is applicable.
3.3 Surgeon Qualifications: The Surgeon assigned to this engagement shall hold a valid Thai medical license, specialist board certification, and a minimum of ten years of surgical practice. Proof of qualifications, malpractice insurance (minimum coverage of THB 10,000,000), and professional indemnity documentation will be provided prior to the commencement of services.
3.4 Cancellation Policy: Cancellation of a scheduled Surgeon procedure by the client within 72 hours of the scheduled date will incur a fee of 50% of the procedure cost. Cancellations made more than 72 hours in advance will be subject to a 10% administrative fee only.
3.5 Regulatory Compliance: All Surgeon services delivered under this Quotation Estimate shall comply with the Medical Practice Act B.E. 2525 (1982) of Thailand, the regulations of the Medical Council of Thailand, and all applicable hospital accreditation standards (JCI or equivalent) in Thailand Bangkok.
3.6 Confidentiality: All patient records, surgical reports, and medical data generated during the Surgeon engagement shall be treated as strictly confidential in accordance with the Thai Personal Data Protection Act (PDPA) B.E. 2562 (2019).
3.7 Force Majeure: Neither party shall be liable for failure to perform obligations under this Quotation Estimate due to events beyond reasonable control, including but not limited to natural disasters, government-imposed restrictions, or public health emergencies affecting operations in Thailand Bangkok.
4. Acceptance and AuthorizationBy signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and financial obligations outlined herein for the engagement of the Surgeon in Thailand Bangkok. This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties.
