This Quotation Estimate is prepared in response to the formal request for a qualified Surgeon to provide specialized surgical services at the Ankara City General Hospital facility located in Turkey Ankara. The scope of work encompasses general surgical procedures, emergency surgical interventions, and post-operative patient management over a defined contract period. The Surgeon engaged under this arrangement will operate in full compliance with the regulations set forth by the Turkish Ministry of Health and the Ankara Provincial Health Directorate.

The Surgeon will be a board-certified general surgeon with a minimum of fifteen (15) years of clinical experience, holding a valid Turkish medical license (Türk Tabipleri Birli¸i registration) and specialized certification in general and emergency surgery. All surgical procedures will be conducted in the operating theatres of the Ankara City General Hospital, and the Surgeon will be available for on-call duty as specified in the service schedule below.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all pricing and availability terms for the Surgeon engagement in Turkey Ankara are subject to revision.
• Payment terms: 40% advance payment upon contract signing, 40% upon completion of the first three (3) months of service, and 20% upon final delivery and acceptance of all surgical services.
• The Surgeon shall adhere to all ethical guidelines established by the Turkish Medical Association and the Ankara Provincial Health Directorate. Any deviation will result in immediate contract termination.
• All surgical records, patient data, and medical documentation generated during the engagement will remain the property of the Ankara City General Hospital and will be stored in compliance with Turkey's Personal Data Protection Law (KVKK, Law No. 6698).
• The Surgeon is responsible for maintaining all necessary certifications, continuing medical education credits, and professional registrations throughout the contract period in Turkey Ankara.
• Force majeure events, including but not limited to natural disasters, pandemics, or government-mandated closures in Ankara, will suspend contractual obligations without penalty to either party.
• Disputes arising from this Quotation Estimate and the subsequent service agreement shall be resolved through arbitration in Ankara, Turkey, in accordance with the rules of the Ankara Arbitration Institute.
• This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by the authorized representatives.

By signing below, the client acknowledges receipt and acceptance of this Quotation Estimate for the engagement of a qualified Surgeon to provide surgical services at the designated facility in Turkey Ankara. The terms, conditions, and pricing outlined herein are agreed upon and shall govern the professional relationship for the duration of the contract.
