1. Scope of Veterinarian Services

This Quotation Estimate is issued by Colombo Pet Care & Veterinary Clinic (Pvt) Ltd., a licensed and registered Veterinarian practice operating in the heart of Sri Lanka Colombo, to provide comprehensive animal healthcare services for the client's registered pets. The scope of work outlined below covers diagnostic examinations, preventive care, surgical procedures, and post-operative monitoring as requested by the client. All services will be delivered by qualified veterinary professionals in accordance with the standards set by the Sri Lanka Veterinary Association and the Department of Animal Production and Health, Ministry of Agriculture, Sri Lanka.

3. Terms & Conditions of This Quotation Estimate

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any services not commenced within this period will require a revised quotation from our Veterinarian team.
All prices are quoted in Sri Lankan Rupees (LKR) and are inclusive of standard materials. Additional consumables, specialised surgical instruments, or extended hospitalisation beyond the estimated period will be billed separately at the prevailing Veterinarian clinic rates in Sri Lanka Colombo.
Payment terms: Fifty percent (50%) advance payment is required to confirm the booking. The remaining balance is due upon completion of all scheduled services. Payment may be made via bank transfer, cash, or major credit/debit cards at our Colombo 03 facility.
The Veterinarian services described herein will be performed by licensed veterinary surgeons registered with the Sri Lanka Veterinary Association. In the event of a specialist referral (e.g., veterinary cardiologist or oncologist), additional fees will apply and will be communicated in writing prior to the procedure.
Client consent: Written informed consent must be obtained before any surgical or anaesthetic procedure. The client acknowledges that all Veterinarian interventions carry inherent risks, and the clinic shall not be held liable for adverse outcomes arising from the animal's pre-existing conditions or non-compliance with post-operative instructions.
Cancellations: A cancellation fee of 25% of the total Quotation Estimate value applies if the client cancels within 48 hours of the scheduled appointment. No refund is issued for services already rendered.
Warranty: All surgical procedures carry a thirty-day post-operative warranty covering suture dehiscence and immediate complications. This warranty does not extend to infections arising from the client's home environment or failure to follow prescribed medication regimens.
Confidentiality: All medical records, diagnostic images, and client information are maintained in strict confidence in accordance with Sri Lankan data protection guidelines. Records are retained for a minimum of five (5) years at our Sri Lanka Colombo facility.
Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved amicably. In the event of failure to reach a mutual agreement, the matter shall be referred to the Sri Lanka Veterinary Association's dispute mediation panel or the courts of competent jurisdiction in Colombo, Sri Lanka.
This Quotation Estimate does not constitute a binding contract until signed and countersigned by both parties. Upon acceptance, it becomes a service agreement governed by the laws of the Democratic Socialist Republic of Sri Lanka.

4. Acceptance & Authorisation

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined above for the Veterinarian services to be provided in Sri Lanka Colombo.
