This folded cardpaper in DIN A4 format was included inside the "NES Super Set" package from Fall 1991. (It might came out in early 1991). It has 4 controller, NES four score device to connect them all. 3 in 1 Game Pak (Super Mario Bros, Tetris, Nintendo World Cup). Shows the covers of issues August+Octobre 1991 as well the "Classic" (1990) promotional magazine (it was updated later (1992), including the SNES and newer stuff) for registered members. Following and previous register cards included in the SNES and Game Boy show some different design. When send in, one could recieve a bimonthly "Club Nintendo" magazine (and the "Classic" with presentations of selected games) and an thin plastic "Club Card" in size of an "Master Card" with your name and an membership number.
